Finance Team Request Form

Please submit questions or concerns about your financial account by completing the online form. A member of our Finance Team will reply to you within 2 business days.

Health and Dental Forms

Each student at Cornerstone is required to provide an updated health form with immunizations as well as an updated dental form before the first day of school on Wednesday, August 27, 2025. You may provide these documents in-person, by email, or by fax: 202-575-0669.

Medication Release Form

Please complete the Medication Release Form if your student needs to receive prescription medication while at school (Albuterol or an Epi-Pen, for example). You may provide this document in-person, by email, or by fax: 202-575-0669.

Kids Ride Free Form

Cornerstone students who use public transportation can complete an application for the Kids Ride Free program. Additional information will be available soon.

Uniform Policy

Cornerstone has a dress code policy to create an equitable and supportive school culture where students can be primarily focused on academic work and relationships, and can develop a unified school spirit.
