Health and Dental Forms

Each student at Cornerstone is required to provide an updated health form with immunizations as well as an updated dental form before the first day of school on Wednesday, September 2, 2026. You may provide these documents in-person, by email, or by fax: 202-575-0669.

Medication Release Form

Please complete the Medication Release Form if your student needs to receive prescription medication while at school (Albuterol or an Epi-Pen, for example). You may provide this document in-person, by email, or by fax: 202-575-0669.

Cornerstone students who are Washington, DC residents and use public transportation can complete an application for the Kids Ride Free program. We will provide an updated request form on this page.

Please note: As of August 12, 2026, we are no longer processing replacement cards. However, 2025–26 Kids Ride Free cards remain valid through September 30, 2026. Please stay tuned for details on when we have received the 2026-27 Kids Ride Free cards.
